After 4 years, 3 months Mr.Pereira's Claim was accepted

But oddly "The Judge" found in our favor "AFTER" the bus company went bankrupt in 2015 proven by an Action news story (proof below) and they took the 5th in 2013 during the deposition, refused to give up the bus safety video, and all 4.3 years after my accident? (something smells fishy there wouldn't you think?) YET, No medical treatment, NO therapy, NO 3rd post op visit (which was in the bill) after the INITIAL fusion by Dr.Jack Jallo of Jefferson Hospital 1-17-2012 when I would of been sent to therapy and/or given further instructions, just sent home with a cab voucher and 0 restrictions,1st one bailed because of Mr. Pereira's health deterioration (coward) and the next I.W.C. lawyers were not out going, just worried about the claim not the reasults or things would be different now, Remember he did have insurance while awaiting the extra lengthy determination. Seen the surgeon 10-4-16 (again Dr. Jack Jallo) he said that the fusion took fine but the neglect and refusal of the 3rd post op visit is the "hospitals fault not his" (had witnesses there to hear that , also where I got the xray image of the bone spurs), I have witnesses who were there. But Jefferson Hospital OUT AND OUT refused him? (their excuse was your workmans comp was turned down and we don't take health partners insurance, ????? after we had to seperate?, I relocated to my daughters?? so I could get it, to get help???)!.... So I also contacted the Bar association to find out if this was standard proceedure, IT WAS NOT! and told I have many civil claims here and satutes are 10 years, between All of the provided journals, So being told we have many civil suits there, So we currently looking for a civil Lawyer "A REAL SHARK!" (And seems like my daughter Sandra may have found a good one, he is assisting her with moving the other grandfather to a better hospital & sadly passed last week 57 years old). I am going after everone who had anything to do with my fall, covering it up and/or procrastinating it to beat the statutes of limitations, thats why now it's civil = 10 years statutes there and for what? OUR TURN! For their actions/decisions at their place of work that resulted in Mr.Pereira's permanent disability. also charges will be filed for involuntary manslaughter in event he expires from any fall out from this judicial B.S. As noted that The Bar Assoc. and others are aware of this situation. This was no slip fall mumbo jumbo, "I BROKE MY NECK and it altered my life forever". Read below starting at bottom of the B.S. that continued after Mr.Pereira proved his claim and since Mr.Pereira preveiled and proof his claim accurate on 5-25-2016 (although still in hell) But to see when the real hell started? The Lies, Deceipts & Procrastination and hindering, the double talk, misplaced forms, Fabrications by O.C. proven lies and what of their taking the 5th Amendment and also not release the security video... ALL OF IT. before Mr Pereira was approved for his claim and what a battle of mis beliefs, mis leadings (thankfully we are photographers and have many pictures to back this claim.) I.W.C and the defendants attorney's did and bought up all fabrications yet Francis Pereira's neck STILL hasn't been addressed, It has been looked at, There have been many, many test, over 60 reports and 17 disk, radiated, contracted Hep "C" yet still no corrections. at least O.C's lies were all for the better as it made the judge mistrust their word, and propaganda exceeded the truth and again he was pushed back to the rear and paper work shuffled. This is really unbelieveable, if he only had this kind of attention in the begining he might of recovered in a much more needed positive manner.
Now this just came in and he is in no shape to keep this up, what kind of games are being played , the 27th of March now the following hearing on the 3rd of May for them not paying after Mr.Pereira did win and was awarded his claim. Apperently it's all mental anguish and pain and suffering unwarranted. Think we all should go down there and see whats up? (We have asked for home care and ignored), no income yet the burden of keeping the home together? So he should go un rewarded, untreated and just observed until he caves in or expires? If he expires many people will be named in a wrongful death suit and will also have charges pressed for involuntary manslaughter that goes for witnesses that lie, owners that run and the legal system who let this circus occur ? Why? Because of their negligence?
THIS SO FAR HAS NOT BEEN A JUST AND FAIR HEARING AND ARE SIGNING NOTHING, UNLESS YOU MAKE IT WORTH HIS LOSS! And the skills, contacts future endeviors are all just that, Santa made up to $300.00 per hour at times. So seen in the beginning was his old friend Steve Bruccolleri’s Firm that dropped him {after going to his hospital room and signing him to contract there.} and the Judge then was an A. Stephano? (see trans scripts from before Spivack & Spivack with Bruccolleri) the actual records are way down the bottom of the stack of records, documents, images, 17 Disc (you know results of being shuffled etc.) Vincent Bonaventura Esq. who the case was handed to, dropped the case because it was to much to handle and after wards , daughter Trish got Spivack & Spivack through a public transportation ad. For starters they had to withdrawal the case and then resubmit it. And if the Judge did choose to not attach it, Did she have a PHD? She can separate an accident injury? Excuse me, People If we have to sue to get the back and neck attached we will. How do you separate a spine? (we think he was built that way, connected skull/spine, was he not?) Because in the Jefferson records it says about the 3 bad disc (so they were indicated, but they chose to focus on the neck Dr.Jallo also said these were all new injuries because they were still pink and inflamed back in Jan 2012) and when I.W.C. Denied his claim, Jefferson hospital tossed him out and refused his 3rd post op visit even after he obtained his own insurance (had to relocate to his daughters and separate from his wife) but still married to and living with as wife and now care giver (lately irritable since Mr.Pereira has no feeling and can not tend to her needs) And which states his daughter is power of attorney, so his wife has no rights at all? Just saying. With Mrs. Pereira taking care of Mr. Pereira, Trish has been a big help and has witnessed much of this case along with other family members and friends of The Pereira Family. This whole case took off on the wrong foot and was never corrected, and neither has Mr. Pereira, he has not been medically, properly taken care of over waiting on a verdict? and now that he proved his claim they still won't do anything for him?, State doctors will not treat a I.W.C. patient, no matter what insurance he has. It is work related. So Mrs. Pereira is left to take care of him with no assistance and it has been to much on her being a woman/mother/grand mother and unable to maintain a job because of taking care of him, was she compensated by I.W.C.? Or Leahy’s insurance co? She is not even a witness and worked for the company at the time? 8 years ? She knew her husbands situation, and no one asked her about her input to this life altering tragedy which includes her? Including husbandly duties which includes being intimate to home repairs to 7 grand children he is the only living grand father to (many died while he was waiting for help), This is ridiculous.

So reading this journal (www.injured.poushe.com), it shows me a man, done wrong, trying to get help from anyone and sent through loops while apparently fighting D.H.S. System to recover his grand children? and still is no where and they want to leave him go now because they conceded (after 5 years of mental anguish, trauma and definitely P.T.S.D. from the lengthy results and after winning his claim after action news broke the story about no bus insurance, (Then they listened?) he was awarded 4 years and 3 month’s later? I.W.C. sent him on an I.M.E. again almost immediately after finding in Mr. Pereira’s favor to which he fell at results and $5.000 bill in the journal, (the 1st I.M.E. Dr. Gopez poked him with a unsterile metal pin (off a table Mrs. Pereira said) to see if he could feel or not, not even under the arms the doctor was surprised. Mr. Pereira at his next blood work showed Hep-C results and it’s the first he ever heard of it (as his wife was negative.) then an I.R.E. shortly after and thus showing a 65% disability ratio. So he’s really hurt? And he tried telling by writing, do to vocal and speech problems resulting from the nerve damage from the surgery, and kept a journal with picture proof. And it is mentioned let him get state insurance and do a mass settlement? and that’s that?
The claimant is in distress with aspiration/choking threats and in constant pain from extended bone spurs that have developed since the fusion due to lack of attention from/since the neck fusions 2nd post op visit by surgeon Dr. Jack Jallo, who mentioned his lower back but was focused on the neck area and still needed to see him for 3rd post op for further instructions but Jefferson hospital refused him and his insurance he obtained and still has for his blood pressure using any excuse, After being awarded his claim Dr.Levin had Mr.Pereira go see Dr.Jallo and after viewing the last MRI seeing the extended bone spurs Dr. Jallo saying "it’s not his fault (when Mr. and Mrs. Pereira recently asked) His fusion took but did not know why the bone was still growing, The 3rd post op visit being declined, It’s the hospitals decision " even offered Mr. and Mrs. Pereira to talk to the hospital admin, which they would rather save for a professional. Mr. Pereira was left unattended when coming out of the anesthesia and in need of the rest room and went, the toilet was broke with water pouring out 2-3 inches of water on the floor, but in his state he still sat to use it, when trying to get up his feet slipped back, he fell forward into the porcelain wash basin with his chest thus hitting the floor, some one came in and they rushed him for a cat scan 10:30 pm and he mentioned calling a lawyer for the bus company when he heard I.W.C. denied him, and the hospital staff thought it was over his fall, putting an observer in his room (which Mr.Pereira referred to as a monk) writing all he said and did until he left the hospital and that’s why he felt why he was rejected for his 3rd post op visit. And recently the ENT’s saying Mr. Pereira can’t swallow without assistance because of nerve damage from the initial surgery at Jefferson and the bone spurs rooted behind the epiglottis and considered dangerous? So go see an E.N.T. So Dr.Jallo will not remove them. So no one will operate or fix it? Seen he's been sent here and there with no results? While laying in bed 24/7 getting bed sores and related problems his back went from 3 disc to 14 or more protruded or herniated disc, he can not feel from the neck down and mentioned it numerous times And the in included Back (which was mentioned in the surgeons report) the problem is his upper and lower areas, rapidly and progressing his whole spine with protruded and herniated disc from extended litigation results pending no actual follow up work or neurological therapy and no medical other than advice & pain killers which he rehabbed to get off of during the wait and search for help.) This is a responsibility that the state should not have to cover it, If it was an act of Labor and Industries judicial system taking a lengthy time to render a decision/verdict they should be held accountable as the results are, the wait was because of witness testimony? but the actual company owners and dispatch employee maintained their 5th amendment right, they also refused to hand the safety video from the vehicle because the driver should of not been behind the wheel, the driver did not have proper credentials and the dispatcher knew it? Was any of these articles subpoenaed? This person (not just a claimant) also had a life out side the bus company he was an Accomplished multi musician, (www.drummers.poushe.com) Sound producer and tech, Stage & Lighting director, Magazine Developer/Editor/Reporter, Magazine layout, Advertising, Media tech, Live broad casting, Film and special effects editor, The idea man & Web and Graphics designer, Event coordinator and was a known Professional Santa Claus who made it a real show at holidays was on the view and other appearances. Supposed to be another court date to access my fathers worth as they see his I.R.E. is 65% so they Conceded and are paying for his home therapy tools and the other things but what of below", all the assets he will loose? The Santa Clause , His Magazines, his webwork? All of his musical skills years of investments that are still monies he made while not working the bus company in the day time and they are going to be responsible if we civil sue them for the lose of assets,

Juan called to inform me that they are paying for the wheel chair, and the other things I need/purchased for home therapy with scripts because my I.R.E. was 65% and no one wanted to touch me in case there was a negative reaction. So they (O.C.) conceded and now want to settle out in a mass settlement, court again 4-27-17 (I don't have to be there) believe me I will have many sideliners around and my daughters for proof of what occurs.

An additinal comment* MP (yea all this after dragging Mr.Pereira through the mud for 5 years, refusing medical, and other things needed to make a recovery, no therapy, no anything, and then concede & just drop it???) and not to mention when Jen D.Kaufman called him to tell him after the deposition he didn't lie, that they had no information on him and he was working with special needs children? That the 2 brother owners and the dispatcher all took the 5th and they refused to provide the bus security video? did anyone subpeona it? and this was in 2013? So why you sit in judgement of what he made just at the bus company what of the other things he did in a different time slot?He worked nights at his own business his magazines. How do you put a price on an entertainer, drum teacher, rounded music career, news paper editor even a well known Santa Claus? Doctors appt to see Matt Trzesniowski my primary, who now wants to view the swallow culture from Nazareth Hospital over the feeding tube being bought up and put me on a soft food diet until my dentures are replaced In which he wrote a prescription for also wrote me out a Thick-It script, which I will use the rest of my life due to ignored bone spurs and nerve damage in the rear of my throat from the initial surgery from Dr. Jack Jallo of Jefferson Hospital.
AND AGAIN! Can anyone recommend a bone spur doctor to help get rid of this pain??????????? EVERY TIME I SWALLOW!And since they don't want to do anything about the spurs or my spine, I am having my P.C.P. write me a script for an MRI on my spine so I can civally sue then to get it fixed.

Ken or Juan did not attend , they sent another lawyer named Jodie so his daughter Trish attended (since he was told he did not have to be there) and they bought up good points and mentioned disc in the back which the doctor (Dr.Jallo in front of witnesses) said in the begining "that there was 3 bad disc from the accident but was more focused on the neck area then on the back, yes Dr. Jack Jallo so his back was never ammended to the claim at that time, well his daughter bought this up and Jodie told her not to worry they would have it added to the case. ?????? So lets get to it, Steve Bruccoleri's firm Vincent Boneventura was supposed to do all this but dropped it after the first hearing yet Steve came to his HOSPITAL ROOM 1-18/19-2012 and had him sign a contract in his room never having the back addressed or added which is now 14 disc. FP -This is why the Bar is involved yes head diciplinarian for the state of Pa Mr. B.Jon Shepard, I don't play, this is my life, and my grand childrens future with their only pop pop and I WILL FIGHT TO THE END for proper care or turn in everyone who didn't includes Doctors who took the Hypocrattic oath and the Hospitals can't refuse anyone action, well I WAS REFUSED!!

Our Lawyer is petitioning the Judge due to the fact that no one from the O.C. defending party wants to pay for therapy equipment, doctors, visits, test, and other things they were ordered to help assist or make Mr.Pereira more comfortable and basically instead got Mr.Pereira's name black listed so the Judge is being made aware and at this time should also know of the permanent out come of Mr.Pereira's health & that they are responsible without doubt that through their (I.W.C's) procrastination and most certainly liable. (if the lawyer does it right from what we were told by the Bar Assoc.)

Have a consultation with Dr.Bruce H. Levin who agreed in front of witnesses that Mr.Pereira has basically been medically neglected since the begining, he quoted that he has been shipped here, there and tested and probed but nothing really done for him kust multi testing. so he said "Poor guys has had 4 & 1/2 years of medical neglegence". Went to Temple and seen Dr. Jamal, Now she did a swallow culture but looked through a trac. window which showed mothing more than vocal cords, unlike Nazareth Hospital that showed the actual side view of the swallow and each bite deflecting down the wrong tube, have to cough it back up. they wanted to have me come back through the emergengy to talk about alternative feeding methods (feeding tubes) when the main problem is the bone spurs behind the throat but told they were in operable, so they want to pre long it to past a critical level, then you OWE ME I.W.C.!, AND OWE ME BIG, YOU STOLE MY LIFE!!!!!

I met my new PCP . Nesbitt ran out never said goodbye after 10 years, well later wacko. I like Matt better yet Dr. Matthew Trezsniowski who is not a iwc doctor but is following close in my case due to the swallow life threat. He is more than eager to assist Dr. B.H.Levin in any way to help me, right now it's the bone spurs blocking my epiglotus and making me choke when I eat or cough. Thanks Doc.

Yesterday I got a call from a lawyer, my lawyer called for me to get SSD or SSI, I already did that, from the previous rep before Juan, Jen Kaufman who hooked me up with a lawyer and I won but it costed me also....see.... So why did I come this far to win my claim to have it turned over to the SSI dept? who side are they on? It really makes you wonder. I feel like I am battleing the people who are supposed to represent me, that are being paid to (had no problem hitting that 20% right?) I think I am going to let Wendy Saltzman from Action News find out why they wont remove these bone spurs preventing my healing process,.

which was yesterday and again a waste of time, he pretty much was and had no intention on removing the bone spurs so the throat retracted and Mr.Pereira could regain an active future, but did manage to argue and scream at his wife after he said to go back to Jallo and my wife commented another waste of time, He screamed in her face and asked her what degree she had? belittleing her and she had already on intending to leave, screamed enough a told her to go (security came) and Mr.Pereira (witnessed) to stay then he got upset to see the Princeton appt page in this journal then asked if any of the test were complete, a new MRI a month after the other one??? an EKG and other things and no script? He never sent them to Juan ???? Yet firmly said NO I'M NOT DOING THE SURGERY, (I could tell before we even went in it was just an ass save visit, he was already had made up his mind that he was not going to do it) ... Just following the E.N.T.'s (Dr.Rowe) orders now... and must contact Dr.Levin.

... and still nothing until I went back to Dr.Bruce H. Levin after another waste of time at Premiere therapy where I should of never went because any movement could impale the back of my throat, the spine doctor there the first time could of caught the bone spur starting but was to busy worried about my electric cigarette So in the investigation it was mentioned that people in my home went outside to smoke proved I didn't smoke so whats the bill for, why wasn't my neck addressed? (another appt was made for Dr. Shaw at Princeton Head/Neck & Spine for 1-24-2017, (see what happens) but GLAD THAT I DID see Dr. Levin from the 1st visit, #1 his testimony is what helped win my claim and #2, he had a clue to what was happening with me. And said light light therapy you have serious problems in your neck now...
I HAVE GROWING BONE SPURS!

Yes, in this timely shuffle around and procrastination (medical negligence) my neck was/IS still growing! Large bone spurs are on the back and in the front is what appears to be a rhino horn looking spur it comes out the bottom interfearing with my epiglotus and the food goes into my lungs (told need a feeding tube soon or better a script to see a Doctor he knew at Temple who wrote articles on precisely what I am going through to see what he thinks a Dr.Jamal E.N.T. for 2-7-2017) and turns up going up my neck and ending in a point like an upside down icycle, it is hitting and putting pressure the back of my throat (I have been complaining about choking when I try to eat and Dr. Levin sent me to an E.N.T. Dr and I knew a good one Dr. L. D. Rowe M.D./Surgeon who took out my tonsils in 1990 who sent me to a swallow culture when I seen him on my birthday 12-29-16.

I went to Nazareth Hospital 1-16-2017 and they would not even try solid foods, they wanted me to come back through the E.R. and admit me for a feeding tube I told them that the doctor who sent me was a surgeon and I would let him make that call so I have a script for a thickening substance called "Thicket" in which you add a teaspoon or more and it makes my coffee, juice, liquid food pudding thick so I "Don't expire from Aspiration as it can happen at any moment and the hymlick might not work if the back of my throat is pierced by the spur" Oh so YUMMY! "NOT" note that I have been mentally toyed with and sent spiraling to all these places when seeing me on the 3rd post op visit might of got me the help I needed. guess I'll never know and since I am an open book people are turning the other way because of the fact I am learning while lame, that I have rights, I see the B.S. in front of meAm so tired of listening to B.S. and the run around, even the surgeon who did the initial work said I should talk to the hospital admin ? door what refused the 3rd post op visit or when I woke up after surgery and no one was there I had to use the b.r. and did in a 3 inch puddle because the toilet was broke in my recovery room , I slid and fell into the wash basin chest 1st see the time of my surgery and the emer c-scan at 10-35 in the p.m. when I awoke alone unsupervised.. My I.W.C. Lawyer who is just taking the claim all this that happened under His watch , my daughter found an ad on the bus all new she took pictures of it saying they do malpractice and other forms of law practice ????? then why are they not helping him with these other cases that have developed from this? hmmmmm my daughter has questions so I told her to see them because they want to have a meeting face to face on the 25th of January wonder what this is about, because I have gone back to plan A and the Dr. who refuses to let this run it's course???? I'm done and thats why I am now BESIDE MYSELF TO GET BETTER, I call lawyers day after day and am contacting 6 and 10 investigating news Mrs. Saltzman and 10 investigators to find out what all the hold up was, Why no one will touch my neck, we have 17 disc and many written reports....BUT NO ACTION. So why did I have to pay for it, Why am I being whirled around in the merry go round of hope and fight trying to get treatment? This is the 1st time EVER FILED FOR I.W.C., Hell I just wanted to go to work that day, they put an unlicensed driver on board . (see the action news article that states they were working with out insurance which forced them to close). I bought a 2nd home and my care giver (wife) an Acura for my appointments to make it easier as transportation was provided but unreliable (On their terms) along with therapy equiptment (I have perscriptions for so I.W.C. should reemburse me the cost, but are fighting it, wait until I realease this journal to Philadelphia, there will be a nice ripple effect I hope!) with the retro pay I recieved when winning favor in my claim.
AND WHILE WE HAVE ALL THIS UN NEEDED DRAMA and while going through this we also were being dragged by the city, trying to take our grand children *we think it was because of the accident case and how we busted Philadelphia's Court Scam* (WE WON THAT ALSO) Journal against D.H.S. So while Mr.Pereira was suffering and fighting his cause of a corrupt bus company, he was also battleing the city for other reasons. which have many civil suits involved. as a social worker lied to Judge Irvine and he put a stay away order on Mr.Pereira (who was in a wheel chair at the time, and never met him but could take a liars word right Saniquah Salaam Hardrick ? when in the journal Mr.Pereira offered abundant proof that they were set up by the social worker Saniquah Salaam Hardrick (he then had a visit video taped catching her in every lie when she said she didn't even know why there was a case - SHE CREATED IT OUT OF HATE!!!!!) see the judges stay away order on a disabled person he never seen, met or questioned just put a stayaway on here-say and now proven that I didn't I have contacted the BAR Assoc. to file com-plaints against Lawyers who think this is a joke and Judges who think time and statutes are not important they are not a right, who said deposition? No one asked me how I wanted to handle it. They just STEPPED on my civil rights to a quick and speedy trial, right?, Here's a stay away order I was never served with, or was it ever marked in my records. I still have a squeaky clean record... So why can't I get the help I need?

Another Problem is Mr.Pereira's legal team sent a letter informing that they are using a ficticious name that the Leahys called him and ask it to be corrected for legal records and such which still has not been followed and he still gets his checks in the name of Frank Pereira when it shows up, it should say Francis M. Pereira.
Here we are at the end of October, My Primary care doctor told me that I was getting meds from 2 places and has to cut me off. We explained what happened and he didn't want to loose his license so No doc, Now No Premiere because they hollered at my wife and were very unprofessional (we thought that all along due to they could not even come up with a ciriculum for his therapy) so I get to live in this pain which I am ready to try street drugs heard this heroin stuff might help, anything to get rid of this nagging neck pain, but 1 more chance to straighten out Premieres mess and I have a visit with Dr.Levin (who thinks Premieres a joke, why he left them) the 31st of October 2016 who will set me up with real therapy (as I use electric drums and a inflatable hot tub for aquatic therapy have scripts from previous doctor Theodore Porter) who wanted me to see Dr.Jallo the initial surgeon who said I have an over bone growth but the fusion stayed but the bone needs to be shaved down, told me to get a 2nd opinion again from Dr. Kim from Temple he then asked us if we wanted to talk to the hospital admin because the refusing was on their behalf not his (situation was workmans comp was denied and they didn't take health partners) so screw Mr.Pereira?. We called Dr. Kim who has not returned the call and Mr.Pereira can not eat or drink now without chokeing, he constantly chokes as the bones grow. Dr. Jallo does not understand why they are still growing (try during the surgery they gave a shot to stimulate the growth after over dosing me on cocktails of morphine and adavan and other things in which I had allergies to but to messed up to talk, after the surgery Mr.Pereira awoke in a recovery room alone and needed the bathroom he made his way there to find a broken toilet and 3 inches of water but still used it as there was no one to call and he had to go upon finishing he tried to stand thus falling forward into the mounted wash basin hitting it with his chest, they rushed him for a c-scan at 10:30 pm that night and released him to a cab, alone and sent him home never calling anyone at his home and no one was there, that's Jefferson Hospitals hospitality for you....
Now since winning his claim the lawyer who said in 2013/2014 in a letter that I was not to go to Premiere Therapy because of excessive emails to the therapist (why did he put his email on his business card?) So I was getting my medication Hydrocone, Diazapan, (for fusion pain in neck and shakes from the spinal cord damage) Ibuprophen (for the swelling in the neck which makes him choke and blocks his airway){Which has been proven it's not the pills it's Bone Spurs! read above.} Lisinapril for high blood pressure which was given by his primary well now that he won sent him back to Premiere Therapy? (b.s.) who also perscribed the hydracodone and diazapan and ibuprophen, thus resulting in Mr.Pereira calling them and telling them he already gets them from his primary, but they sent them any way. The law firm stopped using I.W.P. and switched to Chem-x who messed up an order sending the wrong medication sending Mr.Pereira off the deep end who was almost 302ed for his behavior, and the doctor Dr. Shapiro from Premiere called Mrs Pereira telling that they sent the wrong medication to tell Mr.Pereira to stop takeing it (explaininng his out burst) then recently as 9/2016 they sent a completely wrong order for a woman so we complained and they were more worried about being sued according to the Dr. the woman from the pharmacy called the doctor and crying, afraid of being sued. So they pulled me out of Premiere Therapy again this time saying I was there for the drugs (before his accident he did not even drink once in a while took a couple brown 200 mg ibuprofen over the counter) so if anyone got him hooked on pain killers we are pointing fingers in THAT direction. And since he has no meds he has had a couple seizures but is going through withdrawl and now emmence pain. and turned back to his martial arts (has obtained a 4th degree Black Belt, studied Karate, Ninjitsu, Aikido, Taekwondo, Kung Fu, Muay Thai) to use Tai Chi (Isometrics Exercises) for home therapy (also because we preffered a Dr they had there before, Dr. Bruce Levin who was no longer in the loop (thank GOD) but had said 2 years ago he thinks he could help my nerve damage so I made appointments to see him) see above as said he left that practice because it was a joke and looking at my records (said in front of witnesses) you have been medically neglected.
In the mix Juan R who now represents Mr.Pereira sent him to a doctor Shaw in Bucks County's Saint Mary's Hosp Mr. Rodriquez said "he used the doctor himself for sports medicine", to see what could be done (nothing again as usual again we heard "If he was treated after the surgery he would of had a better chance, now has a massive bone growth in his neck" and that was that, never got a script to see any doctors or anything, never sent a script to my lawyer. The Doctor seen was already not in the zone and knew nothing really of my case. I don't care what degrees are involved, this is the school of hard knocks where we have proven a merry go round of B.S. Never had a call to come in and who is this Susan?
Below is the results for the I.R.E. and then was sent a letter to see if I had worked or was self employed, go ahead PMA Group rub my face in it you will have your time. You held up his medical for 4 years, we seperated to get him insurance to get him therapy which should of been given at his 3rd post op meeting with Dr.Jallo (who we recently just seen 10/4/16 and said it was the hospitals fault not his) and the 3rd post op visit never happened. most of his condition is your fault PMA group (medical negligence). but put him under investigation for not being in a wheel chair that an elected official ordered him to, (not a physician). (also Doctors, even with insurance will not touch a workmans comp case) So you wasted money on that huh? BTW and the fire dept. wants to question Leahys, representations D&O investigators methods as there was a fire in an abandoned dwelling close to the Pereira home, why? to catch him not in a chair? the female investigator was parked right out side their home another investigator pulled up and talked to her, he came from the direction of the fire neighbors said. Did you not see page 1 Before this waste of time did you know that he was lifted from that Judges order by his physician? and for bed sores, muscle anthropy and basically putting him on ice until they could find a way around this. We think it was just prolonged cruel and unusual punishment towards the victim. (the script is there also) So PMA Group, waste much money on that one?.

and since we took him to the hospital and he got a c-scan to make sure his neck was ok in which the paper work said there was a cervical sprain. We had the bill for $5.000 (seen below) was forwarded to his legal rep as I.W.C. sent him for the I.M.E. To which was responded with now he had to go get an I.R.E. (the results are in a slide show above) this whole thing now getting rediculous I am seeking a civil lawyer to go after everyone of these people for their actions at their place of employment. I have already befriended B. Jon Sherman a diciplinarian for the PA BAR ASSOC. to start these processes and to name people who led Mr. Pereira through hoops.
Even SSI in which he paid an attorney for in which was reffered through Jen Kaufman sent him a letter after he informed them he was awarded his claim.
Then Mr.Pereira recieved his rescheduling appointment and we went there and he was barely looked at then after pointing out how his feet were blue and showing circulation problems as his feet turned blue. After seeing this the doctor asked him to try to walk (well it was obvious his legs were a sleep and he fell) the Dr. ushered us out of the office.

Here is the trip to ARIA Frankford division closer to our home, This also was utterly a joke and to make Mrs Pereira leave his side when she is his care giver to help him walk, left her crying in the waiting room afraid he would fall again (this is their asshole security company)

(glad they didn't find me in menapause glad they also screwed up and didn't xray my wife since thats what the urine was for (pregnancy) instead they threw her out because they missed with a needle and she cried from the bubble that lasted 3 weeks we called the Hospital Admin everyday for 3 weeks over the service they provided) and forgot to do the brain and had to go back.

As shown on the 1st page how this drug out for 4 years all the procrastination, lawyers dropping him and having to get new representation thus meeting with my present lawyer Ken Spivack then paper work being misplaced anything to keep it dragging (see page 1's letter from Jen Kaufman) Better yet here The Leahy Bus co went out of business in November 2015 as shown by the Action News Article on the 1st page for not having insurance (wonder if I was working under them conditions as it was a shady company) also about the time Jen Kaufman left the Spivack firm and replaced her with Juan Rodriquez now assigned to my case. The Judge then found in my favor 2-25-2016 ???? over 4 years later???, Now get this after all the evidence I provided she went with... The driver (who didn't have a license or other needed documentation and should of never been behind the wheel) and other bus attendant stories didn't match 1 said I fell in the morning the other said I fell in the afternoon. but we won?, what did we win? more grief! My claim was found in my favor AFTER the bus company went bankrupt November 2015 and Leahy's took the 5th in 2013 during the deposition because they knew Nick should of not been in a drivers seat (which stepped on my 6th ammendment right to face them in a court room in a 6 month period, they still never seen me as of today) Leahy refused to give up the bus safety video, (read above scrolling up) Yet her honor can mention the "Grotesch unsightly looking scar on my face" (circled in the finding of fact page) why I now have a 12 inch beard took 4 years to grow to cover it.
The Judge mentions my "Gross, hideous, scar the is on my neck now" who can put a price on that? and they want to study "what pushes people over the edge?" look above there's 1 way!
