This is clearly proof that what you have been doing, hacking a disabled persons evidence and clearly by what you have put him through he has proven that yous have things here you don't want mentioned, well I for one am pressing charges on many of you. I am a witness to what he has been through. From my point of view, The litigation, both sides have been turned into the Bar Association for criminal cospiracy. Police have been notified We are pressing charges, as far as Jenifer Kaufman, we want her arrested for witholding evidence, and for stringing Mr.Pereira having him fire his previous law firm and for breach of contract along with Ken spivack for never releaseing the stroke information and conspiracy along with Jenifer Kaufman who obtained her own law firm and actually came to my home had him fire his lawyer and whipped out a contract then to which she with drew because he would not get out of the past and asked about Dr.Brody's report, and found out he had stroke during his accident 2012 and didn't find out until April 2021 and was never notified or the rest of the family , Ken Spivack who sent him to Premiere Therapy, since your doctors found it from Carl Contino's establishment, You personally or with associates WITH HELD the information (evidence) we also want to press charges on, This is under investigation by The P.B.A. , Our hosting provider POWWEB, And we are in the process of filing Police reports for witholding evidence and for mental anguish to a disabled person and all of you for Medical negligence, You have to be mentally unstable to put someone through this. he actually wil be seeing his primary care physician on Monday who also will state the obvious as he has the forms sent by Dr. Brody in 2013 which you all chose not to admit into evidence. and never told us of the stroke after Mr. Pereira lawyer admitted she seen something about his brain back when she was with the Spivack firm 4 years ago which proves it was a cover up. WE were informed recently meeting an old co worker July 17th 2021 Omar Shariff Lister (contactable because he has more information against the bus company) AND LOG 5 ---->, (So Judge, William Benton keep thinking they are reasonable.) I still say they are questionable. and running uninsured 5 years after is questionable and the reasonable is now perjury to what ever story he said in April 21st 2021 hearing since they kept running an illegal business after I couldn't return. They went under different names like TLC and a few other fake companies, just to show they are not good on their word so I say reasonable is out of the blue, especially since I was not present to recant the truth (I alreaady knew that or we wouldn't be here, Think what you want Judge Bill Benton) as for Omar Shariff Lister who also worked there? his statement is at the present area, the bottom of Log 5. (my wife trained him to drive a school bus) He had an accident and because of the fake insurance, he was sued because the bus company was running on fake insurance, he claimed so he ended up being sued, He is still paying off the law suit to this day, that they placed on him?) In order for him to keep his CDL license. Which is not fair so I sent him the Action news clip that stated the Leahy Transportation Company was closed Nov ,2015 for lack of insurance so I take that as they knew of Ms. Nicole Johnsons accident after mine, her attendant (was medivacked to Hahnemann hospital after being rear ended because the bus she was operating stalled out going down Allegheny ramp on I-95 (Hunk of junk should of never been on the road) While PMA Group insurance company covered them when I worked there and since been fighting me.)

Then my Primary Care Physician found Dr. Brody's files in a box in his off (explained below) And when I asked my attorney because as seen on log 4 & 5 she pleaded with me to look at the present and the future and stop going into the past? Which set off flags, because she threatened to drop me if I didn't (Log 4 & 5) WHY? (Proof of text screen shots Log 4 & 5) WHY? is because she was with the other firm when they found it, Spivack & Spivack who sent me to Premiere Therapy where Dr. Brody did an EMG and had me go to Nicks, Open MRI and noted the finding but no one never told us just kept stringing us along.. My recent P.C.P. Matthew Trzesniowski found the files that Dr. Brody sent to my P.C.P. at the time , from the previous P.C.P. who left abruptly in 2016 when Dr. Trzesniowski took over the practice over (and he found the files from Dr. Brody sent to Dr. H. Nesbitt) called me explained it we went over it and then he said "WHO DROPPED THE BALL" It wasn't dropped, it was a cover up. (prove me wrong?) and I called her on why was the evidence withheld? She then dropped me August 6th 2021, after she came to me, had me fire my old firm and then tried to lowball me to impress the judge? or PMA Group. (Log 4 & 5) everybody wakes up to pee in the morning nobody here is GOD!. And are replaceable, Why did I turn it in to the Police? Because of witholding Medical Evidence, Medical Negligence of a victim (and is a crime) because there was different types of evidence that was not put into my claim, why it's here in my journal to prove I am not a liar and my family friends (support system helped)

WHY? That would of made a big difference in my medical treatment, had they released it which I was FOR 91/2 YEARS and still am refused. My lawyer had to many excuses and expected my wife and myself to find a way to get the test or appointments done or get her the results (thought she was working for us?). Well the last IME 2021 , she said she would drop me if I didn't go (instead of going for the hernia surgery) was to an I.M.E. Dr. Levin (who would blow this out of the water with information he should of never gave us (as a witness was there and I voluntarily went and set up an MRI Brain and Cervical spine MRI with and without contrast, At a hospital that my oldest daughter told me about (results on Log 5) and my wife, she was pissed, not to mention that it matched the argumentative phone call with my lawyer who told her that 4 years ago she seen something about my brain (she would of been with Spivick & Spivack back then ) and my wife said (over a speaker phone, witnesses available) My wife freaked on her and asked why didn't anyone do anything about it? Why was it not put into evidence with his statement. The lawyer Jenifer Kaufman hung up.(see log 5) after that she always reffered to my wife to me as "THE BITCH". and would talk to noone but me, in all of this to present day never once did they ask anyone to testify on my behalf (hence , the Journal)

#1. You came to me, in my hospital room, had me sign a contract, then you delegated the task to Vincent J. Bonaventura, Jr. who 9 months later who dropped me , you refused all calls, I marked this as breach of contract, I signed on with you!.

(All accusations can be confirmed by Dr. Matthew Trzesniowski, as he had to pick up all the pieces) You proved that by saying "You seen something about his brain to Mrs. Pereira on a speaker phone with family present which started the thinking to get a REAL MRI, Not one of Temple Hospitals doctored up results", or was it this Kim Veon who's name is on half the dockets but we never met her? but 2013 I met Spivack, we were the Munsters. (don't count your eggs before they hatch, ass hole) Thought I was just going to die off from the stroke or not remember? It affected my motor skills not my memory, AssHoles!, But he also falls under the BREACH claus, you delegated the task to Jenifer Dana Kaufman. who left the firm in early 2016, replacing her with Juan Rodriquez which also kept it rolling 3 more years. (but never lied to me)

For the whole incident , (which was never even reported.) She had 3 accidents in 2 days (I was in 2 of them, un hurt)The Bottom line: Nick asked me a question about my Santa appearence, didn't like my answer and popped the gas pedal and made me fall going over tracks.. I already had bad blisters from switching Santa boots and sneaks everyday (holiday season) other than that healthy as a horse for a 50 Year OLD! And I never missed a day either just 2 mornings for court...there was another attendant onboard, It was Nicole Johnsons relative (another driver) her brothers, girlfriend who kept asking if I was ok. (I was beat red and tingleing) Also Margaret Merrero another attendant who left from the 2nd accident with Caroline (back hoe on 611 ripped through the back of the bus) Marge never got unemployment or workmans comp, My wife also witnessed this accident from her bus, I also want Nick (already passed away) and Dave terminated for negligence and prosecuted for lieing and trying to cover it up and the Leahy's Owners that backed them....

So how did my fall happen? look below.. the driver punched the gas and hit the trolley tracks and the bus dipped and I went air born and fell into the back of the seat head/shoulder first. whacked my knees into the metal bar . Trying to compose myself flung across the isle and the corner of the seat hit my lower back on the bend. I got up and sat in the closest seat, another attendant asked if I was ok..

Pereira (was never asked for credentials or drug tested & no C.B.C. or Child Clearence) I was a bus attendant, I attended special needs children also my daughter Elizabeth Pereira worked as an attendant along with her friend Melinda Suarez also Elizabeth rode with Nicole Johnson and was only 17 at the time,. Melinda was 18. (2 years ago) Elizabeth also gave pop a break at guard duty, she even knows he never had a day off until it got to cold to sit outside and they then bought a camera, I was still working so they recorded me on my guard duty. Both and Melinda and Melinda's mother Linda also witnessed the fall in the pothole fall heading to Spivacks office and a few other neighbors... but since they kept me drugged up and never told me about statutes and you only have 6 months to file a claim the now cannot collect un-employment. and I was put on the books in late September 2011, mind you just asked for ssi number. but did work there since 2009 (no real info, trace cashed checks at Bobs check cashing at 2777 Kensington ave for any Leahy Transportation checks about 6-7 employee names will turn up (5 off the books) and on me? anything paperwork wise turned in after the 5th of January (my incident) is in admissable. they kept all bus attendants off the books for years. I was here for 4 years had I been on the books would not be fighting for my wmc, or unemployment THEY OWE ME a lot for non payments for favors also.... (Remember these are special needs children, if the parents knew...(whewwwww) probably be a class action suite against them. Any wacko could of been there with there kids, I never was cleared (but I knew I was cool because I do it for Santa gigs every year) and proof of them witholding work from my wife until she told them if I was suing or not.. pictures worth a thousand words!
Prooves they are a crooked run company who pays out of pocket for services (Marge Merrero witnessed a state police officer taking a wad of cash off Joe Leahy... who then tried everything so she wouldn't turn them into L&I ... ) She has documentation to prove her case. Many investigations found half the busses un useable. Also they do their own bus repair work with unlicensed mechanics (family members & 1 licensed machinic so they dont get in trouble.) most buses are just jimmie rigged and a bad spider and MOLD Problems. My wife contracted a (non contagious) pink eye virus that she couldn't get rid until almost 7 months later...

I need Kens statement in writing, he says he is just going to handle the WC Claim, ok but other lawyers wont believe it. So I asked for him to send it in a letter head. Listed below needs to be addressed,
Fell in a city pothole meeting the Spivack law firm (Now remember I spend at least 85% of my last 19 months in bed so I dont do anything to make me totally paralized) So... after my previous lawyer dropped me, not having representation my daughter contacted Ken Spivack (very nice and professional guy) well I fell in the pothole I twisted my left foot and landed on my left butt cheek which hurt my hip & that's all that was (but it's the citys fault they neglected that area for a while as you can see by the images of the falling dwelling (house) behind it.) so you can tell I missed the appointment. (I want to make the city responsible for negligence & there was also an ambulance involved to that hospital. (need bill needs to be paid) Deb requested my neck scanned because she was just worried about the fusion so that is the scan purpose, the bus incident was almost a year before that. I dont have anything to hide my falls started after the neck injury on the bus, because I am numb shoulders down, feels like walking on stilts, why last June I was ordered to a wheelchair when I almost fell in the Work mans Comp Court. before the wheelchair was delivered, I then also fell at the Criminal Justice building 13th and filbert 2 x's where Police and Court officers turned their backs on me and offered no assistance told to take in ambulance to hospital because my grandson Cloud Cram was abused and I had to be there for the little fella, after that it came and I love it then my new grandson Avion was born from daughter Elizabeth & the Elevator doors closed on me during fire drill and damaged my wheelchair sprained my left ankle *AGAIN* (pothole 1st) possible torn ligament at Pennsylvania hospital and was given meds without being asked of alergies to which I was, also had to make a police report because someone there was using my social security number...never seeing my new grandson went home to be stuck on S.E.P.T.A. Platform from no elevator service, now in the dark about my situations including the complaint about my previous lawyer the Bruccoleri firm who breeched my bus fall contract saying my wife wouldn't listen..she didnt sign the contract I DID!... My wife has filed Chapter 13 recently to save our home, we already paid & filed chapter 7, 2 years ago and a recently found a 20k lean was left on the address (the reason we filed with Matt Lazerus at the Eric Jenkins firm, *very good firm* We bought it to his attention and he fixed it, like I said reliable, and here in Philadelphia it's hard to find a good lawyer Eric Jenson and Ken Spivack are jokes to me. So the banners below are friends, family, top notch services that do their job), and etc.... statue of limitations are going to be exceeded because of negligence, trying all I can, even to make a new Physically challenged easy links site to help Challenged Individuals.... I interviewed with the P.M.A. group investigator, P.M.A Group insurance agent was questioning neighbors so I hobbled out and confronted him...investigator for pma checking fraud seen me and realized I was messed up.. He asked about the accident and said Oh my you (I) need therapy my hands and arms look depleated where am I getting therapy so look out for this stuff..He had a scar down his back showing me a operation he even had to sue for..told him aint bout the money its about getting back on stage...he told me and deb a lot of personal shut off the recorder information ..ask deb said his report will get me rolling. He THOUGHT I was getting workmans comp or unemployment told him aint got a dime anywhere in almost a year and a half. so he said his report will help and the sooner the better. Very nice gentleman.
I went to an IME where their doctor saying everyone has a back like mine and that I dont need another surgery. OH! and the place where I go for therapy is a joke, and my doctor is a little over dramatic...(my opinion...good then switch bodies with me see how much you like being trapped...) Even said I had surgery on my big toe ???? I'd like to know when?... hate friggin liars..Seen my wife go through it. She was a Leahy driver (But NOT MINE)

AUGUST I also need to have my neck fixed as the new doctors stated Jefferson performed a "Botched Surgery" on my neck & that has to be corrected. (the fall in the bathroom from the leaking toilet right after my surgery didn't help because I was alone.) they booted me out after they were told work mans comp was not covering it, sent home 2 days after surgery with a collar and cab voucher. Seen 3 times by nurse and therapist and then nothing until now.Then not seen for follow up surgery, insurance not accepted. They say they have witnesses that seen me stumble, well of course if I have blisters on my feet from switching shoes, sneaks & Santa Boots daily. So let them bring up their none medical degree holding witnesses, It's just waisting more time I could be getting better. Saying I could of broke my neck from a little stumble? & that they have witnesses? ( I have tons of proof I was in good shape! ) Each one that pergers them selves will be SUED & charges will be pressed for lieing in court like "Wheres your medical credentials that makes you an expert on walking" or their going down for pergery, defimation of character, slanderous remarks, lieing under oath etc. & Wheres the bus video tapes? thats law right? Nick admitted to my wife and a few other employees that I fell but he didn't think I got hurt, where are the incident reports? because there are none, like the camera films...How Convenient! also I am documenting the whole incident story to publish and / or proof of the situation as my health has been failing and my family can continue my battle if need be, I am a journalist.

November 12th,
I have had no money since this injury occurred from this incident and now poverty stricken, Now facing surgery 2 on the back of my neck due to failure of the first surgery. I recieved a 2nd opinion from Dr. Bang Su Kim of Temple and Dr. Kim knew right away the problem was damage to the above and below disc of the fusion and spinal cord shrinkage which a 2nd surgery is immenant may or may not improve. (due to spinal cord injury createed from the actual fall inury from this incident) It would help straighten out my balance problem and a few nerves re attached, so set up for MRI in 2 weeks and letting Dr. Kim do the proceedure as he seemed like he knew me, and was right on the problem also VERY Confident, all the things I was feeling, he said and explained it, that I could see it and the channel theory is one that I live so communication was excellent..and so my neighbor around the corner IS walking his dog and had the same surgery? and it is so much closer to home, wonder why I wasn't took there from frankford hospital and taken to Jefferson? they were way closer than all the way to center city...but lookingf at surgery 2 in the back of my neck this time...Told him to keep Premiere informed of everything as I want to continue therapy and Dr. Kim agreed afterwards I will need INTENSE Physical Therapy so....
Now November 15th
And didn't have to go to the hearing all done by deposition anyway., really weak seeing a throat specialist the Nov 18th or 19th have to call they want to go into the back of my neck and raise my spine while shaving bone spurs from new unusual bone growth, asked to find the hormone used for bone growth during my 1st surgery. The judge is waiting for one of the trans scripts from the deposition from the bus company, all this and the owner took the 5th (refused to testify) and Dave Cybuk also (dispatcher) because he knows the records there are either non existant or written on a napkin somewhere, they put up 2 bus attendants who I never met and Dan my old driver before Nick who said "Nick should of been paying attention to his job and not what Frank looked like" Some one mentioned the toy bag hurt my shoulder (glad they said that because I have had no feeling from under my ears down to my feet since this injury occurred from this incident) so that prooves I was ok then. What do these wackos think holding this up? the driver had a different story too. WHAT ABOUT THE CAMERA?

A hernia or 2 grew in the last 2 years getting in and out of bed. but found out on 1-31-14 from 2nd opinion doc Well not quite what I expected? after all the Melon-Colly I looked at the MRI video my self (with the doc) and seen 2 1/2 inches of spinal cord damage but it's stable there is fluid around it but sliding up and down you can see the narrowing but there's a pocket behind it (odd) to where it beds to so They see no reason to operate ????? after all this waiting ? saying that it's permanent from the blow on the seat. So just start intense therapy after the hernia surgery, Dr Tom Santora Temple (he did one for me in 2007, good work! see him on the 7th Feb) so deal with the 2 hernias from lifting in and out of bed 2 years (looked at disc from 2 years ago, not present so occured from laying in bed 2 years waiting) Operate, remove, heal, then vigorous training (therapy)....dont forget the disk above and below the fusion are bulged. So is that the fall in the Jefferson hospital bathroom? see I.M.E.. that could of done it because I did nothing for 2 years but lay in bed?

Thanking all at Premiere Physical Therapy but needed to be closer to my surroundings, An awesome Therapy place, just to far from my home. Thanks for the help and equipment you guys ROCK! and Today I started at Progress P.T. around the corner from my home and today I returned with regiment home work very cool illustrated guide lines and reasons. I see them 2 x's a week they are sending their own company van for me and working hard at my recovery. I may not be able to feel from the neck down ever but my mobility will get back to 70% percent with hard work and looking at 8 weeks or longer of a vigarous work out. I have the want and spirit to do it and it was seen by Jesse who is my therapist. Returning to my primary care physician Doctor Howard Nesbitt seen him and got the green light to work out and also keeping the services of Doctor Bruce Levin for Pain Management. While still waiting on the judges decision, in the mean time helping Tom Wolf Democrat for Governor with Internet Campaigning in the littles ways that I can help out, just it takes a while now so at my own pace is good he said, and he mails me appreciative letters. I never thought being stuck here in pc world I would get into politics ? Go figure (always had a little bite for it) but he is a real nice guy with the same views I do, Same realistic ideas also (I always thought they should pay for the extraction) so it works out. and he has my vote :) I have recieved letters from 4 mayors and a few campaign mgrs who were asking for written permission to use my ads and telling how Tom played an important role in them getting to their goals..Kind of cool stuff if you ask me and if he wins this could be a big help to me doing a disabled function for kids off my Santa side, he is in to child advocacy so that would be a plus! Well got a copy of the sent findings to the judge (While I still try to get SSI.) In the witness for them testimonies, The yo-yo's , there were 3-4 witnesses for them? all admitted I fell of a sort (I had a small tea and packed cup cakes that didn't spill in a bag that landed on the seat) they all said coffee and some even said I was wearing a Santa suit? One stated I intentionally fell? (like I wanted to be like this all my life) and I had a flea market booth every weekend up until Christmas so I could do my day job and since finally on the books did not go for an out of state contract... Even ask Kathy that works Progress therapy, she knew I was the flea market Santa.... So QUESTION? Why would I have a coffee (stains,suits cost 800 bucks) and why would I jeopardize making a quick couple bucks at Christmas which I wait for all year (there were 3 witnesses besides my self the lady, the driver and the camera ) that was not available (company took the 5th) 1 witness (attendant) has been misleading the clients and others including my self with an alias name because we all called her another name Michelle Anderson and so did the teachers of the school we did. Also the prevous year I was Dan Andersons "School bus attendant" for a juvenile detention school on Front st. and being off the books is legal for that position? I jsut see they all purged and said it happened before Christmas LOL If so then the Driver only had a CDL he just got that week (but was driving a month before, oooops no camera footage but teachers knew him by name) (70 years old) and no passenger endorsement (means can't have passengers on his bus) failed test 7 times in 6 months right before this, it's public record ha ha ha, So what to do with the copies? hmmm what to do????? It happened the 1st week back to work after Christmas (Jan 5th) because the driver asked why I still looked like Santa and told him I didn't see my hair dresser yet (which a witness also mentioned cause she quoted me saying I didn't see my hair dresser yet) (at a red light) went back to my seat, light changed he popped the pedal and hit the Erie ave trolley tracks bus dipped, I fell I can go on but everyone already knows cause I ain't got nothing to hide. Have an appt with Dr. Bruce Levin on the 31st. Therapy tomorrow which is going good we figured the cold weather really hinders me but other wise they say I'm a trooper :) Thanks Clare, Kathy, Jesse, Dave, Julie, Lillian John & Juan. Got Myself S.S.I. , by the wife and myself going to the Aramingo office and doing what we had to do. (see Latest MRI link to left) Don't know what O'Shea's Office is doing? and haven't heard from them either, (We take credit for getting this through our neighborhood SSI Office) I think O'Shea office is for S.S.D. Well today had and IMA Scheduled by a S.S.A. Rep J. Delucca for SSD also gave a copy of my latest MRI (link left shows permanent spinal cord damage!) The I.M.A. is kind of like an I.M.E. for W.M.C.) so I don't know whats going on but it's happening before my case out come???? Hiring another lawyer(s) to sue workmans comp for exceeding my statutes of limitation (see back after Elevator, June, then Feb 18th, after Jan *statutes time*) and making them responsible for the bus fall, and the post operative bathroom falldue to leaking toilet also to do something about ther other matters not handled by current workmans comp lawyer. (See List added months below) Since they complicated and went way past time statutes in my situation, It was bought to their attention right after the surgery, when opposing lawyer used my wifes medical records not mine 2.4 years ago) After I seetle my case of course, but at least my lawyer can put it right in after my verdict. Recived my first check and got my grand kids Easter stuff, yet un able to give Vivica, Cloud and Judian theirs until my wife can take it to visit. (I still respect the 1st court order and stay away, and so does everyone else and I never said that to that woman even my daughter knows I didn't talk to her since I video taped her lies!) Today is my grand daughters birthday she is 6, I hope I get to say happy birthday over a phone call and Clouds is on Wednesday the 23rd. hope to do the same there. also my last day of therapy and the kids would of filled in the gap keeping me active outside (doctors note ) but they are not here so what do I do now? feel very trapped, betrayed and in a situational depression to a point of very depressing thoughts, I am considering getting help for this, I DIDN'T DO ANYTHING WRONG! You Embarrased Me with your decision and labeled me with out meeting me and didn't know I was disabled and issued a stay away order I never was served? Today is my birthday, haven't heard anything, freezing to death , no heat can't afford it. SSI boosted my check 38 dollars whoo hoo ??? (Frank Pereira, my father has been deceased since 7-4-1995) Well as of today, I am only 53 years old, disabled due to company driver error putting an attendant behind the wheel, and then almost lost my home to sheriff sale due to lack of income not once but twice (going through it now, feels like the Twilight Zone) and for what? for my journals that keep me to date what's going on in my world?. All because I witnessed a civil worker and then taped her being corrupt? leaving a door open for a very strong suit? So this is how they cover each others backs? Well we took 1st last night I knew Tom Wolf had this from jump street, Why I volunteered to help back him been busy with him and his wife Frances through the internet, Yesterday was also Deb and I's 26th anniversary (since we met) This Friday will be Debs 43rd birthday and still out of work taking care of me.. I am/We are all still fighting for my grand children because I have the city backed into a corner (notice I never been served yet, or arrested or anything else in all the stuff I been airing all year?) and I see the kids on weekends? so Who's lieing? So this is a joke and drawn out drama and just unbelieveable. . So any word yet on my WC Claim? almost 3 years? have any of you heard anything at all? It's Nov. 5th and still nothing... well from my last letter over a month ago.... They refused to testify, and will not produce the video so what else do they need? I made a 100% percent clear journals as I have nothing to hide..., (which I just up dated) It just doesn't make sence... there's nothing more they can do for me this is it ???? and I wait....

Januart 8th 2015 and still waiting, Finally relizeing no one gives a hoot, someone might have to actually do their job, Hey I just wanted to go to work that day...The driver (who was not legal and just an attendant like me) drove off before I was seated. I fell when the bus dipped on the trolley tracks and broke my neck., I just want to see/copy of the video I know the bus had a camera. It's the Law! Then they took the 5th and refused to produce the video *which is law on a school bus* and after 3 years of leading it on to permanent damage and still waiting on a workman's comp verdict! ? ? this just looks hopeless anymore Think I'm going to talk to some lawyers about how to get results. Still told there's nothing more they can do for me and this is it, and I wait....

February 8th 2015 and It Started!
My claim is again being sabataged by low life scum who creep the web taking old pictures of me being Santa and saying they are recent?? ...and they wont testify or produce the video that by law should of been there so they are now? Well I have all the proof that the pictures are old, also statements and the photographers statement and the photographer claims theft by them as they never asked or purchased the photos. So all the pictures are in admissable or they will sue, I had permission to use them, to re post, Showing I used to have a life.... And again they are lieing saying the pictures are recent. But this time with damaging evidence against them, pulling up a text message showing they with held work from an employee because they would not give information on my case and where a witness tells a friend she over heard the guy driving appologize to me. and the wildest is they had a P.I. and another P.I. watching my house 2 days (neighbors witnessed it) and the 2nd day a fire broke out on the block and she documented it all in her report, the start time, my actions coming to the door with my cane (my life story for a year and a half since I and my wife got me strong enough to get me out of the bed and wheel chair, although I still have no feeling from the neck down.) So I was checking on my wife who was out helping neighbors. One of my oldest friends and neighbor got a lot of smoke and water damage. Am I not allowed to be by my door? my bed is right on the other side. She said in the report she had video of it (many things) and since she was there when it started until it finished, It's an open arson case (I know firefighters) so I let the fire dept know there was a lady in front of my house spieing on me and wrote it all up in the report and said she video taped also, now they want a copy of the report and to contact her for questioning...see Gods watching... "smh" Nice try, next.... by the way? the investigator has video, wheres the one from the bus cam that day?
Investigator Sat through this documenting me? Did she even call the fire dept.? They have the report and are looking for her now.

Well after going over Mr.Pereira's information packet that proved nothing other than a last ditch effort and waste of time. He had a great active life and will loose a lot of potential income? That this whole investigation was that he was supposed to be in a wheel chair and could not walk? Are they friggin crazy? He walked into the court room (with witnesse's) The Judge ordered him to a wheel chair, and the results, he was getting bed sores, laying, sitting and his doctor ordered home therapy (even gived him the equipment) as he was to extensive and a liability to touch since being rejected by workmans comp. Well I hope some of the stuff below, and all the media reports on NATION WIDE SCHOOL BUS COMPANY'S being the lowest trained in the transportion field, that their is a problem and he is truly a victim of it, but to of suffered this long without help or something else could of helped that could of went further into his situation? It's intolerable! His spine has never actually been addressed, just depleating, Until he IS permanently stuck in that chair, In which he was COURT ordered to, not by a doctor. They, like us encouraged him to walk since he could, yet none of us know how he's doing it? Man some one needs to wake up here. It's like he's balancing and moves like robotics. Well Mr. Pereira had unbilcal hernia surgery in 2007, has mesh in his stomach and can't lift over 10-15 lbs since, wife does everything. The flea market was setup by wife and family and he was fine through out Christmas season he just couldn't lift heavy articles from previous hernia surgery in 2007, If the school bus company had it's paperwork on Mr. Pereira then they would of known that. They never did a C.B.C. Child Endangerment check, nothing just asked for ssi number in September for Willow Grove (was a must?) Mr. Pereira worked there on numerous occaisions (about four years guard, attendant, cleaning-etc. *off the books* but paid under company name personal check, as seen by "TOP LINKS", They did encourage Mr. Pereira to drive, like his wife (she did not want him to) So he repected her wishes due to first time working since "weight restrictions". The drivers could not do runs in Jersey because most buses did not have DOT numbers on them at the time (see records), Wife was a driver for them for 7-8 years, different route. The man driving who was an attendant (for Mrs. Pereira, before Mr. Pereira ever worked there.) like Mr. Pereira and had no business behind the wheel of the bus, correct? Even if he was in training. D.O.T. as CDL certifications are done through them. Also check on the accidents that put the attendant behind the wheel. From Mr. Pereira's driver, Caroline (who kept her grand children on the bus) he was in accidents days before with her. Why the attendent was put on driving, They also were encouraging Mr. Pereira to become a driver, against wifes wishes. Mr. Pereira actually fell with the attendant, driving over trolley tracks (was not seated yet) *School Bus should not proceed until all are seated* & also and do please take note : Mr. Pereira has never filed for a workmans comp claim in his life. Even when he broke his toe at Precision Cleaners in 1998 he said my boss treats me good, He is still paying me and the bills are covered and never filed a claim. It's a shame what they are doing to this real nice guy who always looks out for others but when he needs help, everyone else looks away. Look at the media lately and they admitted that School bus drivers recieve the most least amount of training in the transportation field after a lot of thrown off violations in the news lately. (Feb 12th on the news woman leaves bus, has melt down, showed BUS CAM VIDEO) The attendant driving Nick, failed the General Knowledge test 7 times in 6 months, (he would gripe about it) He was retired so choose to try to drive a school bus? Tuesday March 10th (Cloud had his tonsils and adnoids removed)

Even disabled I managed to get clear across the city with my wifes help to be there for my little man at C.H.O.P. The staff was great, He looked pale and he was a little redundant looking when I peeked in but seen me and brightened right up. (his father never showed his face) Even the nurses seen how we quick bonded, poor lil fella was hurting came sat for a quick picture and seemed a tad rambunctious from all of us coming at once. Then he gestured to me, had me lay on the bed with him, until he was asleep :) GOD I miss them! The only smile I seen was when I got there, and he wouldn't let me out of his site (GOD, I love that young man, all of my babies) but he needed me I could feel it and now I want to help all I can in my situation from my injury. Even the nurses had 50 questions how I can walk, how can I not have hot, cold, pin prick, pinch or feeling sensations? etc. (same old stuff they all want to know), I don't know! Determination? for Justice? what ever! I just know that no licensed surgeon/doctor ever ordered me permanently to a wheel chair, Instead send me home from neck surgery, in a collar with a cab voucher when workmans comp rejected my claim, wife and neighbors all witnessed it. Because wife was not home, no one to recieve me she pulled up to find me hobbleing out of the cab, then neighbors got a chair and carried me in. then went and almost fell in court the judge ordered me as a all risk to a wheel chair, but with occuring bed sores from sitting, Doctored ordered me to home therapy for circulation. Script and equipment walker, weights, cane, other things provided from Premier Therapy.
Monday March 16th (Went to my primary care doc, had blood work done 2 weeks ago)
And because these STALLING asz yoyo's want to keep this game going, while Mr. Pereira tries to get help, Is tested positive for Hep C ??? WTF!!! Mr. Pereira barely goes out, don't drink, don't do drugs, don't drink off people, had to be from one of the hospitals because he didn't have it 3 years ago, will start back tracking tomorrow, but BOTTOM LINE! I Hold the procrastinators of this mockery of Justice responsible, If he had the proper care from the beginning from the "FALL ON THEIR BUS @ WORK!" this would not of happened so I hold all of them responsible. O.C. & Their Investigators, W.M.C., Leahy Bus co., for kicking this around, lieing, trying to admit false information. & takeing up valuable time it would of been to save me with nonesence. and for my civil rights being stepped on. (Speedy trial! my ass!) over 3 years now.. and going on 4. When all this is over, MY family has all the information!!! ..|..

Thursday April 9th (Went to new Therapist)
Sanjay Gupta is where Mr. Pereira is going for therapy and pain management on Allegheny ave. The last 2 visits they used a tenze unit on Mr. Pereira's back a small one first and yesterday a big one and both times were MAXED OUT for 15 minutes and Mr. Pereira felt, NOTHING! His skin and muscles moved like a breeze on a pond and he didn't flinch showing the nerve damage. He asked when are you doing it? Therapist replied with "It is at it's maximum? wow he actually has no feeling" they exclaimed, how is he able to do what he does? Because he did sheet rock (plaster board) when he was younger learning how to walk on stilts, it is the same now, he says it feels like he is on stilts, the stronger Mr. Pereira's muscles get, the less chance of falling. and all because this is sooooo drawn out. or he would not have so extensive nerve damage or Hep-C.

DO NOTE THE BEDSORES!, WHY HE WAS TAKEN OFF THE WHEEL CHAIR AND MADE TO DO HOME THERAPY AND TO EXCERCISE! YET STILL HAS PROBLEMS, AS FAR AS NOT USING THE CHAIR, ANOTHER LIE CALL BONEL AND SEE ALL THE REPAIR ORDERS!

Tuesday April 14th (Waiting on the 30th)
We are still keeping Mr. Pereira active and he has a new tablet with voice activation (gift from a friend) Just no social networking. Watches Hulu when awake, We are waiting so we can finally get him the help he needs and apperently it has not effected his knowledge, he is still blowing minds with ideas as well as doctors minds trying to figure out his nerve damage.. HE WILL NEVER HAVE FEELING, NECK DOWN! They should of never pulled this crazy crap putting him under investigation, and trying to make something out of nothing but a Judges ruling but removed by his doctor due to bed sores? Basically over another lie? looks like they are under the eye now. He is a real nice guy to be taking advantage of and a victim.

Thursday April 30th (Court today? Waiting on the call.)
Today was the hearing to let the O.C. try to admit their fraudulent investigation. Funny wanting to add more lies to the others, I CALLED their attorney out on. (using my records saying they were my husbands) and the owner and other workers refuse to testify?????? 4 witnesses whose stories didn't match. (were only 3 of us on te bus, they also refused to show the bus video WHY? 2 reasons 1. It would show Mr.Pereiras fall and 2. It would show Nick driving the bus and he was only an attendant like Mr.Pereira at the time, he never should of been behind the wheel he was only in training, FLAGS!!!! (See all the school bus incidents on he media? this is one also!) Any way I called the office at 4:30 pm as it was geting late and no email or call, The machine said that the office was closed due to a funeral will reopen at 12:00, quite understandable and on the 29 th we were called in the morning and they told me, he ( Mr.Pereira) did not have to show up for it. It bought back a weird memory, in 1995 Mr.Pereira's father passed away, he had a lawyer named Ilene Kalman (9th & Cherry) who handeled the estate, after doing all the work, setting up the trust, equally dividing to the 3 of us we went to call about going in to sign off the office answered the phone and said, Mr.Pereira we are sorry to inform you that Ms Kalman passed away last night, I was floored she was so cool and funny, it took the firm 3-6 months to actually settle with us after that, funny just mentioning that because when I heard the recording all I could think of was that call Mr.Pereira made that day and he wept, I didn't know he felt so strongly about his council but she was so full of life...yet others like The Bus Companys Council do this to him you will have your day!

June 30th 2015STILL WAITING! So any idea yet whats happening? Not like anyone ever calls or emails updates. Today makes 60 days since the none admission hearing. So that means that the same 2 pieces of paper they gave you on Mr.Pereira and then The Bus company refusing to testify over 2 years since they took the 5th (refused to testify and release the bus cam tape) and still he waits for this dragged out scenario, Thus makes this case stagnant and we were told when Mrs. Pereira called out of concern for her husband a while back that Ken Spivack told her not to call back there any more. (She was upset at her husbands failing health. and after Stephen Bruccoleri Law Offices firm dropping him with out warning (after they came to his hospital bed and had him sign a contract.) because he wouldn't lie and told the story exactly how it happened, (see lawyer links left nav top) Hey there was a video camera on the bus and they refused to give up the tape (still) Mrs. Pereira did not know what to do and was upset, (Do they not teach compassion at the law school you attended?) and to say anything other is Slander as she never harassed the firm, barely talked to it. She was told That Mr.Pereira was his client not her for her not to call back there. So being put in her place, this is why she has not called? Like that huh? and legally married? so she was stripped of her wifely duties at a lawyers call?, So why his health has been failing because she can't help him because Mr.Pereira's legal representation won't communicate with her/him/them? You think you can treat a clients legally married, concerned spouse like that? Don't think so and then keep this case dragging this long?, some things not right. Mr.Pereira had a funny feeling why he has not been contacted, and after breaking down on fathers day and talked with his wife, asking why she has not helped him with finding out whats going on with his case and then heard about his wife being told to stay out of it and he flipped. Whether Mr. Spivack likes it or not they are married, she has been the one taking care of him, got him out of the bed, helps him dress, runs his errands, Prepares his food, helped him with his therapy, not Mr.Spivack, (his firm represents him (if you want to call it that) and was dropped from the firms therapist because of excessive nerve damage they didn't know where to start, (because his back was never addressed) and then dropped him for overly emailing the therapist? Sending emails because it was the only way for the client to communicate? because he didn't understand what was happening to him? and could barely speak) and this fall on a school bus case with such little evidence and the O.C. refusing testimony he should of been awarded there on the spot back 2 years ago when they had the deposition. So this is 60 days since the last hearing 4-30-2015 a total of 3-4 wastes of time hearings that had given O.C. enough time to make false investigations and continue to harass The Pereira Family. So maybe it's time WE contact workman's comp to find out what the hold up is since no one is allowed to call the office. Mrs Pereira made that clear when disclosing she wasn't allowed to call there when Mr.Pereira could of been getting the help he needed and deserved? Also Mr.Pereira never gets calls in, (we print his phone records every month). This game is over, your being called on it, So what is going on? Have you received a letter from the Judge for 2 pieces of paper work submitted by Leahy on Mr.Pereira's records and The Leahy owner and dispatcher who put the attendant behind the wheel also took the 5th refusing to testify? (a couple years ago?) Mr.Pereira surely did not receive a letter from the Judge.. So Mrs.Pereira is not allowed to call the office to check on her husbands case at his request, Then maybe it's time some one looked into that and can get him the answers he needs for this, since no one is able to from the family side. You gave P.O.A. to his daughter Patricia whom you called Marilyn? And when his wife was there? you know that does not stand, do you have proof she was not able to deal with his crisis? It looks like she did pretty good and he's more vertical and a little mobile thanks to her help since she was the only one, You know another doctor is not going to treat a workman's comp case! The journal was updated also, (happens a lot because they won't do anything.) You might want to see because it's about to get released to the media if we don't hear something soon Mr. Pereira is a VICTIM! And the last entry is basically this letter. So what is the meaning of getting it moving faster for this poor guy, if it hasn't moved at all other than a bogus hearing for a bogus investigation in a couple years?

Kaufman!* Mr. Pereira fought to stay alive to prove the bus company killed him and tried to get away with it. He can no longer have a normal life and requires assistance now from others which in his eyes are as good as being dead. "Hep C" coming into the mix during his case only helped progress it.

Still no news no ome ever calls, just an email you''d think 2 pages of info of me from the co and they took the 5th 2 years ago this would of been over...STILL WAITING!!!! And in the middl of it my partne wrote me off our project, with hope of returning but still to see this and know it's not my fault and still waiting for a determination almost 60 days after the last briefing hearing April 30th 2015 when the Judge refused anymore evidence, so leaving what was presented the year before that. So I been waiting over a year and a half for just the same thing? thats what I been waiting all this time for and also to paranoid to go out since they have me thinking my own shadows watching me when I did NOTHING WRONG, I AM THE VICTIM HERE but seems Like I'm being prosecuted. I was also told by other counsel that when they tookthe 5th 2 years ago and refused to testify I should of been awarded immediately and probably would of had the treaments I needed before it becoming permanent! So whats going on???? I'm not the only one wondering now, hmmmmmmm Waiting for me to die,, aint happening just living in HELL , Which I, in Civil proceedings plan to do as well! This is not fair I could of made a multi million dollar company here!

The Pereira's went to a fishing spot for his hand and wrist therapy, They noticed a Police inpound lot for illegal vehicles next door and low and behold there is a Leahy School Bus there in the middle of the lot, thus proving the Judge made the right choice favoring Mr. Pereira and why the owners who pled the 5th because they still run a slum bus company almost 4 years after Mr. Pereira's injury. Now I am PISSED, the owner of the Company that got me my wheelchair met with me, They will handle the chair it's under a lifetime warranty. Thats not whats got me angry, 2 years ago the bus co took the 5th, Owner and dispatcher refused to testify showing they had things to hide, (I know because I witnessed them) I AM AN OPEN BOOK! & I still am waiting for this to settle to start Civil proceedings and I was told by countless lawyers, students and advocates that I should of won when they refused to testify. Now my paper work disappeared and was found it in the briefs section again (which we already did and the judge was hearing no more of it) It then was supposed to go for determination, INSTEAD IT WENT BACK TO BRIEFS! But my atty found it. She sent the attached letter to the Judge hearing my case explaining my case. Now I have been under investigations/observance by this PMA Group but never told them that at the medical supplies store, The OWNER of Bonels Medical voluntarily said in front of my wife, also Maria (a worker there) and I quote "That this is a PMA Deed (to cover my paper work, they are known for trying to get everything tied up so time runs out!) " and then well they copied the letter from my wife because they call the firm and get a run a round. Now they can reach Jen on their own. In the mean while I helped my daughter win her kids back attached are the custody papers proving How I succeeded and it cost me 0 dollars because I had all the proof I needed, and fearless to back it (Thank you Uncle Sam) now it's time to go after these yo yo's. and PMA your going to regret meeting the Pereira's 4 cases Mr.Pereira won while waiting the out come of his own claim.

And we are still waiting, and some legal friends mentioned to our family, A Labor and Industry Compensation Judge ordered him to a wheel chair? (thus accepting resposibility) yet still could not render a decision? so Mr.Pereira is ordeered to a wheel chair and no payment compensation or medical approval yet? For the last 2 years he has been broken, borrowing to stay alive, faceing sheriff sale on his home 3 times, No other doctors will treat his case because it is a workmans compensation situation. So this wait which is leading to mental anguish as Mr.Pereira is paranoid everyone is watching him because also during this time The PMA Group put him through HELL! (we are all witnesses to that), We all feel he is getting the shuffle, And because of almost falling in the court room, her honor realized he had a serious injury, and The bus company refused to testify or provide the bus-cam security video and then tried every sneaky thing in the book while the waiting continues... Do they realize when he was ordered to a chair (even if for his own safty), yet no means of support or medical assesment?) the judge taking pity on him, he should of been awarded then? that's what we were told.. We are about to call the Bar Assoc. to investigate this claim, this is wrong..This is not justice something is wrong here.......

Letter to Judge 7-1-2015 Yes we are keeping everyone up to date, while making flyers to distribute at a protest being planned, WHY? Almost 4 years the OC refused to testify, no bus cam security video, a bus attendant driving not a certified driver and The victim hounded by investigators, multiple emails from reporters So why not give the what they want? #THE TRUTH! and pass out nice flyers with pictures and a statement. and a URL link to this site so they can see it for their selves. Think about it people, if the victim was lieing don't you think by now some one would of counter sued, even a slander charge for all he has stated but nothing, 4 witnesses with different stories (only 2 were present) which shows Mr.Pereira is telling the truth and this case is just being dragged along. Still waiting...... Family is considering The Bar Association to investigate the delay in the "expediant" decision since the bus compamy refused to testify... and the claimants been suffering longer over 5 months since the last briefing when her honor was allowing no more evidence.
Since ordered to a wheel chair, Since the injury occured in the begining of Jan 2012. So the care giver(s) and claimant, still are waiting for a rendering of a decision and more than proved their point. Will close this after it's decided.

There is no reason for this to be dragging this long, lack of evidence from the Opposing counsel or other errors are just hindering the progress of the claimant. Just proof that there is def a problem in our civil/legal system that needs addresssing And what of the lack of important employee documentation not available and 4 witnesses when there was only 1 along with the owners and dispatcher not willing to testify or produce the bus security camera video. Yet the claimant has shown ample proof shown through out this journal that he did many task for them (picture albums available in documentations). When a school bus company can get away with these types of work ethics and violations and for how long? Good question? for how long and how many employees besides the claimant have been violated. (seen others listed throughout journal, The Leahy's told them ,anyone talks to us and they are fired, told by ex co worker Omar Lariff)

While Mr.Pereira still suffers, no real diagnosis, and now permanent nerve damage, no relations with his wife since the injury, Mr.Pereira almost lost their home 2 times due to back taxes and drug back from winterization even our neighbor who won a contest for our home to be remodeled for his situation, and this was 3-4 months ago and been waiting (because of damage from Super storm Sandy) but I'm not allowed to talk to reporters? So of of course I'm getting frustrated and untolerable and so are friends who are tired of seeing this drag out when it was a person/claimant proved his point, he's still waiting and getting worse. And those who are assisting with bills and such as $700 a month from SSI is all I get. My wife is my care giver and will not leave me un attended And they still didn't start my house and been waiting a few months now. And To my lawyer dude no one threatened anyone. Don't care what double talk you do over a phone and speaking while someone else is is bad manners, Understand there is a chain of command to follow, I WILL get fair justice, figure it out. Still waiting, no Christmas for me or my family? Let Jim Kenney, other state and city offices aware of this, this morning... Now Mr.Pereira's home is being renovated for free and to handicapped guide lines and code by E.C.A. removed rooms, REDID ALL THE WALLS AND CEILINGS, laid new floors and are still working, they added a first floor bathroom, ramps, other helpful accessories as wall rails, toilet rails etc. rebuilding everything (New Journal Coming showing the new changes to our 50 year old property) and FREE! so how is it others can see that Mr.Pereira is a victim, yet a decision is so long?, yet a Judge can assume responsibility and order him to a wheel chair? by her ordering him to a wheel chair, is that not taking responsibility? WE THINK IT IS and he should of been compensated and medical provided along with that order, instead he lives day by day which is not fair. Why does this concern us? WE ARE ALL HIS CARE GIVERS! and he is in a 4 year debt that is still growing trying to stay alive.....

? Still nothing, moneys really getting tight. At lease the house is coming along nice thanks to E.C.A. and their expert team. His sister would of been 51 and on the 23rd will be passed a year and never had closure. We are going crazy in debt reached out for additional help and Action news investigations reached back could not under stand "If they refused to testify, then why the waiting? and ordered to a wheel chair by a judge not a doctor? and put under investigation for it?", We have been asking the same question? so let's see what this brings, while waiting the pics of the house repairs and work will have a link as well as E.C.A. This Leaves questions although He Prevailed with the truth, How is it that November 2015 the school bus company went out of business and to our ignorance ny personal ongoings

(Due to dealing with bad timing and messed up lack of communication in the E.C.A. house renovations) And it even made the news yet never notified? found out from Mrs Pereira's ex co worker Nicole on a pass by April 09 2016. Also mind boggleing and he is now now after 2 years welcomed back to Premiere Therapy for therapy?, and they sauid they never complained about me that I just stopped coming in ? I sit there wishing I was dead while watching all these fraudulent claims going to the window pulling out their canes or what not but you can see they are way off better than me , (yet I sit and wait) and therapy and leaving with no problem but I really need it and I'm put on ice? still can't make a therapy list to follow, just show up and work out? yet they were concerned before about the "Extensive Nerve Damage" and could not figure what to do with me or what I was capable of. what if I rebreak my neck or snap a nerve? just pump me up with meds, yet still never really evaluated by a physician? but asked to speak of this ordeal? So he does what he knows his drums and swimming which both use every joint and muscle in his body , the battle trying to feel the things he is holding or doing. (he tells us everything) And what of the decision with the wheel chair order by a I.W.C. Judge? When it should of been therapy if anything was orderable as Mr. Pereira was awarded nothing at the time then the wheel chair order was administed by her honor (at the time), out of pity from a stumble as Mr.Pereira did not have a grip on his disability standings, thus hurting him more than helping him and that contributed to Mr. Pereira's current situation?. by a lengthy hold up for an investigation of why he wasn't in a wheel chair?, the doctors sent him home in a cab with a collar which probably didn't help the nerves because he fell after the surgery (see records c-csan 10:30 pm January 17th 2012) because he was left unattended and was still under the influence of anasthesia, (he had a right to an ambulatory transfer after such a surgery) And since they can't figure what to do with the situation, use what he knows and make it theraputic in which we figured our selves and with doctors approval Home Physical Therapy muscle flexability and gripping and reflexes along with psi control, Hand Eye coordination, ANGER MANAGEMENT! So we purchased home therapy equipment to try to get him comfortable as the mental depths of what all has happened are starting to be a problem. And Vickie Eckert fighting it was even more of a delay. All to which we will send the receipts and the perscriptions to the legal dept. who can send them to IWC for re embursing, As he didn't need this type of work before, it was a natural gift he is trying to win back so they are now necessaties dor his personal therapy since no one is confident enough to make a call on it.

Found out on leap day yesterday Feb 29th I proved my claim wirh the judge finding in my favor

& that My lawyer no longer works at this firm, It would of been nice to be informed Jen. But No goodbye anything huh? well it's a job I know, So welcome to my new rep who said there is a determination hearing coming up on my answer machine, Hello Juan Rodriquez nice to meet you and hope after this determination hearing that this is the last entry and there shouldn't be a hearing it should be either a decision reading or rendering. Today March 3rd 2016 I recieved a letter from Industry & Labor I won , proving my contest = The driver and other attendant (witness) said different times of my fall. so now to get a copy to my lawyer.

At deposition Leahy Transportation refused to testify or turn over the security video and went bankrupt 11/2015, sSo the lawyers keep it dragging while they make a run for it? you can run!, BUT YOU CAN'T HIDE!, So Explain? OPENING REMARKS:

To make this journal short, (proof through out the journal) Even though Frank Pereira was found in favor of his claim, the Judicial tampering by Labor & industries I.W.C. (paper work shuffle) and timely procrastination and multiple attempts by the insurance company to not pay for his care and family doing all the physical care and legwork to get information knowing from previous bills that the insurance company had no plan on paying wether he was awarded his claim or not. like the rest of his treatment going back 7 years. And being told countlessly by his Lawyers,reps and other so called concerned "were trying to get him help?" The fabricational excuses are so transparent he has over 12 unfilled major perscriptions seen later in the journal and now an inoperable hernia on page 3 from pulling in and out bed then a Mr. Dan Duffy donated a hospital bed that he was given and couldn't sell because a man had passed away on it and there is a perscription for a new one but they again won't be financially responsible, yet he's the victim with life altering changes that hinders any extreme therapy if he was able to reach that level. Now through out this claim are many, and different types of passing the buck, pointing the finger but never anything for him just all they wanted him to do to not have to pay him lies that are so transparent just by reading and seeing the dates and other proofs or why would it take 7 years trying to get him help? Still to this day 2-20-2019 we are fighting for it and Labor & industries I.W.C. Keeps letting it happen after numerous complaints and arguments has proven 1 thing to us that Labor & industries I.W.C. IS one of the largest Legal SCAMS in and upheld by the UNITED STATES. As a victim and being pushed in circles and listening to multiple fabricated excuses now sees that everyone pays into it (It's a law), but if you get injured at work you have to get a lawyer, and fight the system to get back what you paid into most of your life when you are truly injured (we have seen workers who are just trying to ride it in our waiting for medical attention.) the journal/diary below and surrounding articles will prove without a reason of doubt that it is a flawed system and just a cover up for the state and country to make money. My wife left ,she was getting angry PATEREK lied to bust my stones she never tested me but wrote me up as having a seizure ???? WTF???, She didn't test me , no eeg so how could she write something so damaging? I made the appointment to a neurogist at Temple, Dr.Pookas who gave me an EEG and I passed WITH FLYING COLORS also conversed about the patterns. When I fainted was because I bought a 2nd home for my daughter and grand children and she had a stay away order on my wife and daughter and I blew a gasket and I explained it was my blood pressure (and Sue the triage nurse confirmed it.) but no one wanted to hear it and now I have to fight to get my license back as I can drive better than I walk 7 years and neglected still able to go to the store no accident record so proof please I had a stroke and your staff is 100% positive I was laying on the floor convulsing (remember my wife was there) or your lieing! (as noted left side below). They Are Responsible To Pay!
Every time you refuse, you are breaking a Judges order (temp law) and when you take it to court and you loose you are wasting our time, and prolonging the claiments needs. We will start Pressing Charges. AFTER the surgery the cut nerves that created PLS and thats nerver damage stemming from the operation (which means the nerves cut under the surgical area eventually die from both ends inward), the same as the disc in his back. This is all fall out because they wanted to use a driver in training, To which we will begin to make police reports and then press charges for neglect, we all have had enough.Mr.Pereira never made a claim before and was a good person mthe last 7 years dwelling on this made/created a barless hostile lawyer. He took on Philadelphia DHS, APM system. So with little help and no counsil and for the love of the kids he , prevailed by proof., even being damaged also put on a stay away order by Hon J.Irvine after hearing case worker propaganda, never meeting me, seeing I couldn't move, but could pass judgement on me off of hearsay? nah! In 2012 Mr.Pereira's claim was denied by I.W.C. thus sending him home with a neck collar and a cab voucher, no one was home when he got there neighbors had to help him into the house, but then ordered to a wheel chair by an I.W.C. judge? back in 2013? after getting legal representation This is input from the claimant, the family and friends & EVERYTHING is documented since it happened with either pictorial or documental proof. This is the journal of everything, The illegal operations of a school bus company to medical neglect, deceiving lies from O.C. a heated investigation for not being in a wheel chair, ALL proved lies from O.C and Leahy Transportation. All proof by the Pereira family, friends, ex co workers. The point is That Mr. Pereira worked before Christmas and was in 3 accidents in 2 days with his previous driver Caroline, she was terminated but they were short bus drivers and put Mrs Pereira's (she had been a driver there since 2006) Her old attendant Nick P. who was her attendant for a few years, behind the wheel as she even testified in the journal as he was at least in training (but not legal and 0 logged hours driving) He went over trolley tracks before Mr.Pereira was seated which caused him to fall and a slight turn that made him fall again. Mr.Pereira told his wife when she seen him limping and made him tell work after the weekend showing signs of instability dropping things, falling complaining of the numbness thinking he was having a heart attack, Work then took him to the W.C. Doctor who took him next door to ARIA Frankford for a c-scan told him of his neck and they were not equip to deal with it, who then transferred him to Jefferson Hospital to a Dr. Jack Jallo who did a botched neck fusion and released Mr.Pereira right after because they claimed "Workman's Comp denied the claim Jan 2012" witnessed by many (why we hold them responsible for all this yet can order him to a wheel chair?) Yet Dr. Jallo claims it's the hospitals fault that I didn't have my 3rd post op visit when I would of been going to therapy. This is also why we feel this is IWC's fault And after getting legal help then Mr. Pereira went to court and was ORDERED to a wheel chair by a I.W.C. judge (not a doctor) without medical or compensation. See Journal Link "Ordered to a wheel chair" because of a stumble in the court room from lack of therapy? If she had not ordered him to the chair and to therapy things might of beendifferent. (which also lead on to other incidents, see Elevator doors! and other Wheelchair problems he never should of had, he should of been in Therapy! He had health partners at the time and still does...) But now last couple years awaiting another Judges decision on determination??? Also after the surgeon Dr. Jallo gave him 0 restrictions as witnessed by Mrs Pereira and daughter Patricia Pereira, The doctor Joked saying "after therapy he could even go bungee jumping" (proved that he should of been ordered to therapy, not a wheel chair because of ones opinion) See ordered to chair link in journal because this only destroyed the nerves leaving them dormant when they could of been saved by working out, had he had the follow up with "IN HOUSE" therapy and not put on ice to wait (wheel chair). The results would of been truly more positive. And then the lies from the Transportation co. legal council, even tried using Mrs. Pereira's medical records from an old accident that Mr. Pereira had no involvement in, and Mr.Pereira bought it to their attention during a mediation hearing before the 4-30-2015 hearing to admit evidence of not being in the wheel chair?? (that a workman's comp Judge ordered him to not a doctor?) taking up more valuable time when he could of been treated and helped. And the investigations from PMA Group and then from Leahy's O.C. of why he was not in the chair?, went a little over the edge to prove nothing! he was lifted from the restriction a year prior ???? (just to take up more time.So a judge can order someone to a wheel chair forcing the claimanent who was still being denied to assume responsibility but not come to a conclusion and to a determination but can order someone to a wheel chair?,, We think he should be rewarded from that day on as by taking the least bit of action even if for their own safety, should also be known as he was accepted by the Judge who ordered him to the chair thus accepting responsibility and all of this since is merely a drawn process without Merit. (because his doctor had lifted the order back in 2014, because of the bed sores it was creating and muscle loss.) Yet they pursued and with many witnesses for the Pereira's and down to a suspicious abandoned house fire all documented below we are fighting back and ready to bring this to MEDIA attention and told not to by lawyers? whats there to hide then? (Mrs. Pereira had to guide the one female investigator out of the neighborhood because she blocked her self in a bunch of emergency vehicles while violating the families and neighbors rights to privacy, filming people who said they will sue if their pictures show in Mr.Pereira's case etc.*(there was also a man in a car who came from the middle street the wrong way which would of lead access to the rear of the targeted dwelling, he stopped and briefly spoke to her ,witnesses available he conversed with the female and left) * They messed with and are still making a physically challenged individual worse and proved nothing but mental anguish and threats to where we almost had to put him in because he would be to afraid to go anywhere, say any thing, while uttering saying "they are all watching me, they are out to get me, them Leahy's all have guns" (which is true) the emotional distress was and still is a big problem.No one should be forced to live this way! (All Pictorial Evidence, Documentation and Prescription provided through out the journal. But since the wait has been so long it has resulted in permanent extensive nerve damage leaving Mr.Pereira in his present state. And Still Mr. Pereira waits for a determination letter from workman's comp going on 5 years after the injury..., that should of been executed over 2 years ago when ordered to a wheel chair in our opinion. Earlier in the 2015 year they played shuffle the paper work which had us back in the briefing pile (proof below).
Mr.Pereira's representation found the error and brought it to her honors attention. and her honor had said she was refusing anymore evidence and that was 4-30-2015 as of July 2ND was up for determination by the Judge (still waiting 2016 now) and 2/25/2016 Found in Mr.Pereiras favor of his claim, although we feel as though he should of been accepted when a I.W.C. JUDGE ORDERED him to a WHEELCHAIR! thus putting any help on ice leading to todays situations, with no help AND only rebuttle from MANUFACTURERS ALLIANCE INSURANCE COMPANY! AND VICTORIA ECKERT/FOGEL REFUSING PAYMENT WHEN HER OWN TEAM AGREED WITH MINE. She's nuts or is she being paid? hey read the journal anything's possible here in our opinion. All backing an illegal company, you wonder why people have problems with Americas Judicial System, It LIES. There is a WHOLE JOURNAL OF INFORMATION HERE PROVIDING PROOF WITH PICTURES, NAMES, DATES, And witnesses. Proved unethical work conditions, skimming, Many break down situations, rigged equipment and tax evasion. They were employing off the records caring for minors and special aid students, hiring under age workers also off the books and a dead mans account (their late cousin or family member) in which all were paid from. Witnesses added their testimony to the journal. The journal is full of cookie crumb trails to nail these guys. Mr.Pereira and his wife Deborah(who worked there 7 years prior to him seen many under handed things go on but were threatened by termination if spoke up.)As they did to Mr.Pereira in the beginning of the year Sept 2011 when he told them of a driver who was not driving safely (They gave Mr.Pereira off 3 weeks for bringing it up, yet paid him $250 a week because was just recently put on the books as the run was in Willow Grove $350 a week. Then switched him to a different bus.) He proved this in pictures, statements and yet he still waits, got his own intsurance by separating from his wife temporarily moving with his daughter and Dr. Jallo refused it (Health Partners) never giving him the post surgery exam ,THUS NEVER CHECKING NERVE DAMAGE FROM A FALL IN THE HOSPITAL AFTER THE SURGERY (See he had a c-scan after surgery?) the bathroom pipe was broke and he fell in 3 inches of water on the floor, his family witnessed this as he tried to use the bathroom and there was no one attending him.. all because he didn't accept the insurance. (all over money? thought a patient couldn't be refused?) (which is wrong, he screwed it up you can see by the pictures hands stiff and nerves not working and didn't want to take responsibility for it. and WE ALL KNOW IT! Just like WE ALL KNOW Mr.Pereira & fact is... He did get injured at work on a bus, Nick P. admitted to Mrs Pereira when Tim Leader was picking them up for work the next day, seeing him limping said he was sorry he didn't know Mr. Pereira fell that bad (yet never filed a report) and why the company refused to testify, because Nick was not legal to drive passengers yet (check his records) even colleagues at work heard of it., but no one asked or subpoenaed any of us only people for the bus co. who still showed Mr.Pereira was ok with 4 different stories by 4 different witnesses. And the Bus company refused to testify and also refused to give up the video tape from the bus we ALL KNOW was on because the red light was ON and it would show 1. The fall 2. the driver who was an attendant having no business behind the wheel and 3. there was 1 witness on the bus besides the 2 of us.. also available in this Journal is the results of c-scans, MRI's the I.M.E. for S.S.I. (awarded in 2014) and other medical files all can be found in this journal, there is nothing here to hide, it's all an open book! The case reads: Frank Pereira vs Leahy Transportation Where it should say, Francis M. Pereira vs Leahy Transportation. They never did any Criminal, Child endangerment or other background checks, no drug screening policies in place, Mr Pereira was also an attendant the season before with Dan Anderson as the bus operator for a troubled youth school by Front and Luzerne. (Hunting Park Academy now) but never missed a day even bought the kids pizza pies the last day of school, then Mr.Pereira worked the yard in thee summer cleaning buses or guard duty for the batteries which were being stolen (check public records) Even with all this trust the Bus company knew nothing of Mr.Pereira other than his wife was one of their top drivers and very responsible, did not even know his correct name calling him Frank because his wife did. And all this sweep it under the rug stuff and while working with minors and these are special needs children, if the parents knew they were sending their children out to ANYBODY??? ..... So basically we all believe this to be the bus company and Injured workman's comp's fault for neglecting his needs when he was injured at work and rejecting him. Also the school board for giving them routes without checking their credentials or none of this would be. Mr.Pereira has been under a house arrest feeling and probably will be the rest of his life (house bound) for some one else's decision. Has he not suffered enough?
No matter what progress he attempts, makes, over comes is solely the assistance of his family and no one else but His wife, Premier & Progress Therapy agencies regardless. And during his bad days, his good days (rare) that it is all due to someone else's decision. Company owner, I.W.C. JUDGE AND OTHERS WHO HAVE NO CLUE OF THE NEUROLOGICAL DAMAGE & Mental Anguish that THIS CREATED, DOWN TO THE LOSS OF A BABY.
